The intake packet is one of the first steps in starting services at Ruby Beach Behavioral Pediatrics, LLC. Please follow the link below to access and fill out the intake packet.

Other relevant documentation that is helpful, but not required, to submit with the intake packet includes:

• Psychological Evaluations

• Speech Evaluations

• Occupational Therapy Evaluations

• Individualized Education Plans

• Any other assessments pertaining to developmental goals
